Tips on Filling
out the Forms
- To navigate to the next field on a form, use the "Tab" key
or click your cursor in the field.
- To navigate to the previous field on a form, use the
"Shift" + "Tab" keys.
- To mark check boxes, tab to the field and enter an 'X' in the field.
- To scroll through the selection boxes, tab to the field and use the
arrow keys.
- For multi-line boxes, use the "Enter" key to bring you to
the next line.
- If you can't see the data in the field on the Adobe Form, it won't
be visible when you print.
- Once the form is submitted, the forms are cleared of all data. If a
hard copy of the form is desired, you can use the "View
Form" links on the confirmation page to print the information you
submitted.
You will get an email message when your claim is
successfully submitted. If you do not get this message, please contact us
at 1-800-327-0686.
Acrobat Acrobat Reader Download |
